Eye Care for Animals is available to assist you and your veterinarian with treatment of your pet's eyes. While you may contact us by phone, we encourage your veterinarian to use the attached forms. This will provide a record of your pet's medical history and any previous treatments.
Please click on the link below and print and fill out the new patient forms. Then fax, mail, or hand deliver it to the facility your appointment is scheduled at or the Eye Care for Animals practice that is nearest you.

Eye Care for Animals offers many options for payment. In addition to cash, personal checks, all major credit cards, and CareCredit, Eye Care for Animals also offers convenient financing through an independent company, Wells Fargo Financial. Wells Fargo offers many no interest and revolving payment plans for treatment costs of $300 to $25,000 Please contact the ECFA location nearest you and ask about the Wells Fargo plans that we have available.
